Planning transition for your young person's life beyond school can be an exciting but challenging time for families.
On this page we have provided links to various information and services which can provide support.
At the school, Year 9 or 14 plus, Annual Reviews we will commence a Transition Plan for your Young Person and introduce you to the Pathways Service and the Adult Social Care Transition Team.
